Did you know you can book appointments and request repeat prescriptions online?
If you would like to register for our online services please call into the surgery and this can be arranged for you. Please visit reception with photographic ID and evidence of your address.
Did you know that you can also view your medical records?
If you would like access to your medical record please ensure you read the following leaflet - if you are happy to proceed then complete the 'Access Request' form below and bring to the surgery with a form of photo ID:
You can only request access to medical records once you have been registered with the practice for 3 months. Please note that if you request access to your medical records and this is approved by the GP you will only be able to view documents from the date that you registered with this surgery.

Online Consultations

If you are unable to complete an Anima online due to digital access, please call reception on 02392 513143 and they can assist you. However, please bear in mind that any requests submitted via reception are placed into the same clinical triage box as all online requests and will not be dealt with any sooner than requests coming into the practice which are submitted at home by the patient.
Calling the practice has shown to be more inconvenient as patients will still have to wait in a call queue to submit an online request, this can lead to a delay in clinical triage of the request.
Please give a phone number that you will be available on as the Clinician will only be able to try to contact you twice. If you know that there is a time that will not be convenient to receive a call, please let us know in the Anima form.
Please do not contact us regarding your routine Anima request, all Anima routine requests will be clinically triaged, and you will receive an update from the practice. Submitting multiple Anima requests will overload the system and create a backlog which could lead to a delay in a response from the practice.
Help us to help you by filling out the Anima request with as much information as possible. Please also utilise the ‘self-help’ options like visiting your local pharmacy to get advice on issues such as Sore Throat, Cystitis etc.
If you feel your problem has become more urgent you can access NHS 111 online via NHS 111 online or telephone 111 for advice
If the Clinician needs to see you face to face, they will arrange this with you and advise you on the process of attending the practice
